BC PHCRN and CPCRN have partnered with Healthcare Excellence Canada to be involved in their CareForward to primary care initiatives. We would like to encourage as many BC primary care practices to participate. We will be working to complete cross country research/evaluation of all participating practices (who consent) for purposes of learning together and advancing primary care practice innovations.

Enhancing Integrated Care

Ready to strengthen team-based primary care models, including virtual care, to provide more coordinated, patient-centered care?

Enhancing Integrated Care supports primary and community care delivery organizations to strengthen integrated, team-based care models—including virtual care—making access easier and reducing pressure on emergency departments.

Around one in five emergency department visits happen because of limited access to primary care. Enhancing Integrated Care brings teams together to share knowledge and implement integrated care approaches that help bolster social and healthcare systems, addressing hospital overcrowding.

In May 2025, Enhancing Integrated Care launched cohort 1, engaging teams from across the country to advance innovative, team-based care models starting in July 2025.

Building on this momentum, cohort 2 is now open for applications, offering even more organizations the opportunity to join this growing movement and drive meaningful improvements in integrated care. Cohort 2 will start in December 2025.


Right Care Challenge

Have a great idea to improve access and ease pressure on emergency departments?

The Right Care Challenge supports health and social care organizations to launch or enhance initiatives that ensure patients receive the right care, at the right time, in the right place—all while helping reduce avoidable emergency department visits.

Participation in the Right Care Challenge is easy and guided by your interests and capacity. After registering, teams can apply for monetary awards that recognize their efforts to address key issues like access to primary care, workforce transformation, or new care models.

Apply for as many awards as you like—the sooner you register, the more opportunities you’ll have. No matter what you choose, you’ll gain access to virtual learning, coaching and networking opportunities focused on issues that matter to you.
